FORM OF THE APPLICATION FORM AND CONTENT OF THE APPLICATION UNDER NO MISCELLANEOUS CONDUCT OF AN APPLICATION SOLUTION SOLUTION

ISSUE OF A PERMIT

 

Article 1

This Rulebook prescribes the form and the content of the application for failure to issue a decision to refuse the application for a license within the deadline determined in accordance with Article 41 paragraph (4) of the Law on Public Roads.

Article 2

The application referred to in Article 1 of this Rulebook shall be submitted on the form “Request for Permission to Issue or Refusal to Reject the Application for Permit” in white A-4, which is attached as an integral part of this Rulebook. .

The form shall contain: name of the body submitting the request, place of number and date, middle part of the request, name of the legal entity ie name and surname of the natural person - applicant, head office of the legal entity or address of the natural person - applicant, a brief description of the application and place of signature of the applicant.

A copy of the application for a permit shall be attached to the application referred to in paragraph 1 of this Article.

Article 4

This Rulebook shall enter into force on the day following its publication in the Official Gazette of the Republic of Macedonia.
